What's almost certainly going on there is that the heat is in fact hot water while the air ducts are for central AC only

Nobody had central air back in the day. The hot water system is 50+ years old, then when they added central air sometime in the last 30 years there was no reason to get rid of the furnace

if i was going to put in full ductwork for AC i assume i'd get rid of all the hot water for aesthetics and simplicity... maybe it costs more than i'd expect.

Ripping out hot water registers seems like a huge pain in the ass. At a minimum you'd have to paint & do a bunch of trim work. Might have to do some flooring stuff too.

Not to mention, you'd be buying a brand new furnace and throwing out an operational one.

Also, I feel like hot water heat is superior in many respects to forced air. IMO it heats rooms more evenly and holds temps more consistent than forced air.
