in that there is apparently rather intense pain during the extraction/severing process, followed by immense relief once the relevant nerves are severed and are no longer reporting to the brain. the pain goes from direct intense signaling to background trauma, and the sudden decline in the intensity of anguish generates its own sort of ecstatic response.

this is why oldschool torturers generally preferred non-extractive/eliminative methods of trauma, so that the affected nerves would continue sending signals. crushing or drilling rather than removal.
