The "realism" in this game is implemented in intriguing ways. Like, whenever you shoot a non-silenced gun (basically anything but a handgun), you briefly alert the enemy to your position. You don't totally give away your position, but it gives them an idea of where to look so you really have to think hard before you pull the trigger. Drones can be useful for drawing enemy fire, and there is one character class called the tank who's main job is literally to get shot at so that your teammates can try to locate enemies. Artillery is also good for drawing enemies out of hiding spots and forcing them into open areas if you have a pretty good sense of where they're hiding
Camouflage is a big deal too. Missions vary not only by the type of terrain but by weather conditions and time of day, so you have to create preset character configs for the different scenarios. If the mission involves an outpost in the middle of the jungle, you have to decide in advance whether you want to move from building to building or stick to the jungle and keep a lookout for anyone crossing the road. Ghillie suits aren't great if you're in a shack next a warehouse.
Also the level of teamwork needed to be successful it's impressive. If you play an assault class you really need someone to do you recon for you. If the other team has a medic you need to find that out really fast and make it a priority to destroy that fucker.
Also, you always have to mindful of where the dead bodies are, because you know the enemy is going to try to resurrect them. A credited tactic is to lure multiple enemies into one area and pick them off one by one so that all the bodies are in one place, making it easier to mine them or have your sniper take out anyone who makes a move for them.
I have no idea whether these things make the game more "realistic" in the sense you mean, but they add new levels of dimension to what would otherwise be a fairly standard 3rd person shooter.
Devs are also good too. I have not encountered any hacking and they are quick to modify shit like thermal vision and drones so they aren't overpowered. Maps are really works of art.
