I would advise you to get the doc to write a formal letter to your new PCP and have it sent ahead of time if possible. The new doc will want to see you first but if your old doc sends a letter, unless your PCP is an asshole, I don't see how it is a problem.
There are horror stories on xo and elsewhere about new PCPs being suspicious and "well that's not the way I do it!" and then being jerks and requiring you to go through a bunch of hoops, but the fact is if a doctor ever writes you a script for adderall and you refill it every month and go through whatever hoops they ask of you, I see no way the docs will stop giving you scripts.

The way I see it, you have nothing to lose with the doc since you're going to be leaving in a month. Just tell him that you're out of the medication and need it every month to function (meaning you take it daily as a good little boy should), and even though you're leaving in a month you need something to bridge the gap. If he says yes you're covered, and then ask him to send a letter about the meeting to your new doc.
You can also just bring a bottle to the new doc but they are increasingly suspicious about giving out the scripts so you might have to shop around. My biggest worry would be a doctor saying something like: "I understand you've been on it for a while but I am not comfortable giving out the script until you go through medical testing."
At that point you go to another PCP.
