I can't comment on every particular product or service as compared from one U.S. location to any international location. Lots of shit is really expensive in Hawaii. Don't know if that has anything to do withit.
But the superprofits to be made in the U.S. market affect whether certain drugs/devices are available at all, anywhere, because the money is only invested if the profits can be made. Maybe if large profits are no longer available in the U.S., European nations will allow for greater profits to be made in their countries, so ensure that development does not slow. Or maybe certain technologies simply won't be developed (or be developed as quickly as they otherwise would). |

Wouldn't US consumers be much better off if the US government was as protective of their interests as foreign governments are of their citizens? If that means that foreigners end up paying more why does that matter?
Even if that does mean the total profit pool shrinks and possibly the pace of medical research is slowed then given that most treatments are not cutting edge and in the US there are many people who can't afford even basic treatment let alone cutting edge then isn't this a worthwhile tradeoff? |

But the problem is that if we start paying less, there is no guarantee that foreigners will start paying more to pick up the slack. Thus, if we start paying less, there is a significant risk that the level or rate of development will fall off for everyone.
Whether that's a worthwhile tradeoff is not an easy answer, especially considering all the other tradeoffs involved in a lot of single-payer systems (wait times, etc.).

In college while studying the global AIDS crisis it occurred to me that the greater the intellectual property protections for healthcare innovation, the more innovation we'd see in AIDS treatment, particularly since AIDS is something that Americans tend to care greatly about relative to the scope of the problem (versus heart disease and other ailments).
At the time, a HIV (and herpes) killing lubricant was progressing through regulatory approvals and needed more investment. But the leftists wanted to permit generic production so that the lube could be available in the third-world equally quickly as the first world. But, by requiring that the company do that, it inhibited the capital investment needed to bring the gel to the market. I argued that we need to offer IP protections against generics for 7 years.
I was rudely told that I was completely wrong and a moron for making such a suggestion. Which at the time I took to mean I misunderstood the issue, before I realized that their goal wasn't to maximize the well-being that could be gained from such technology, but rather to spite those who profit from healthcare and to punish Americans for being wealthy.
They also refused to acknowledge that condom sex is inferior and believed that African men resisted condom use due to cultural pressure that wearing condoms is unmasculine and thus didn't see much utility for this gel. |

The best systems (Germany, Switzerland, France, Australia, Singapore etc) have mixed systems. The government pays for comprehensive coverage for everyone via public hospitals and subsidising doctors visits and pharmaceuticals and medical devices. That way everyone is covered and nobody is hit with bankrupting medical bills.
On top of this you then have the option of taking out your own private health insurance which lets you have more choices when it comes to doctors and private hospitals and lets you queue jump for non urgent treatments. In Australia about half the population elect to do this.
