Milan is quite an interesting city in its own right but not nearly at the same level as the other two and since the international airport is way out of the city you don't need to go into the city.

I wouldn't. He's probably going to spend 95% plus of his time in the three inner cities and the last thing you want or need is a car in an Italian city. Just go by train.

I wouldn't rent a car unless you were doing a driving holiday across the Italian countryside visiting lots of small towns and historical/scenic sites. Even then you need to be a reasonably confident driver able to cope with a stick shift and maniacal Italian drivers on narrow roads.

What we used to do was set our lunchtime or end of day destination in the GPS and then just drive in that general direction taking whatever detours seemed interesting.

That way the GPS is always calculating the best legal route from where you are to your destination which is really helpful in Italian towns which are usually a maze of narrow, one-way and blocked off streets.

GF is near fluent in french though which is great when we're there.

This reminds me of a funny story about my husband. When we go to Eastern Europe I usually do all the translating because I speak Slovenian fairly well (which means I can get the gist of other Slavic languages) and I did 6 years of German in school which is a very common second language there.

So when we go to France he's all "now I can translate because I did French in HS" and I ask "how many years?" and he says "two" and I respond "I did five years of French". LOL.
