Family forced me to get vivitrol. Having severe chemically induced depression

Walking my dog seems like a marathon. My brain feels in a fog and I can’t think. Feels like a car trying to turn over and not catch. Insomnia and feeliking like I’m outside watching myself. Everything seems weird as hell.
Basically I can’t even work and idk do I tell them I need Fmla leave? |

It prevents you from getting high on opioids and apparently works with alcohol as well.
