First, there are 4 ways you can insert an implant: inframammary (under the breast fold), periareolar (incision around the areola), transaxillary (through an incision in the armpit), and transumbillical (through the belly button).

I am a fan of the inframammary. Yes it will leave a scar under the breast, but that scar is small (particularly with implants) and the scar will fade over time. It's also in a more unnoticeable part of the breast and doesn't impact aesthetics nearly as much as the periareolar incision. Some people prefer that incision since you have a natural color change at the areola, but I think that's misplaced, and imo that incision almost always ruins the aesthetics of the breast.

My second favorite incision is transaxillary, BUT that is a much harder incision for creating the pocket in which the implant will reside, and so it needs to be done by a skilled surgeon. Also the public can see an armpit scar, so that has to be considered. Finally, the size of the implant (silicone) can be an issue with placement here.

I am not a transumbillical fan. It limits one to saline, requires a special surgeon, and can leave track marks for a while on ones stomach.

You can place the implant 3 ways: subglandular, subfascial and submuscular.

The benefit to subgladular is that it requires the least recovery time -- like a couple days. The downside is that it looks the fakest and is generally the lowest hanging implant. When you think of 80s breast implants, those were subgladular.

Subfascial is below the fascia but above the muscle. Slightly longer recover time, but more hold and less sag.

Submuscular is my preferred method. Yes the recovery time is longer, 1-2 weeks, but the breast looks much better. A woman will get much better upper pole fullness (the top part of the breast will look more filled out and perkier) and the muscle will help hold the breast up and will prevent sagging.

Implants generally come in 3 profiles: moderate, moderate+, and high profile. A moderate implant is wider and flatter but has less outward projection. A high profile implant is narrower and sticks out further from the chest. Mod+ is in the middle.

The profile of the implant is typically determined by taking into account two things, (i) the width of the chick's rib cage and (ii) her desire for cleavage vs projection. A wider, flatter implant will give more cleavage, ceterus paribus.

Typically petite girls will opt for high profile implants because they don't need nearly as much width to achieve great cleavage. A chick has to be careful here though -- if she doesn't have a narrow rib cage, using a high profile implant can leave a chasm between the tits and make it look like she has baseballs attached to her chest. If a chick with a wider rib cage wants more projection, she should go mod+ with a larger implant.

You can get implants in two textures: smooth and textured. I believe smooth is supposed to have less risk of capsular contracture.

Implants come in two shapes: round and "tear drop". The tear drop is supposed to look more natural (mimicking the actual shape of a breast) and give more upper pole fullness, but they are known to shift around a bit, so for that reason i'd advise anyone going for a natural look to do so by getting a more moderate profile silicone implant under the muscle.

Saline vs Silicone

I am a big fan of silicone. Today's silicone implants are nothing like the silicone implants of old. They are known as "gummy bear" implants because they don't leak. if you cut one in half with scissors, no silicone will come out. Silicone will look and feel MUCH more natural than saline, and will nor cause any rippling on the side of the breast.

There are some downsides to silicone: (i) I believe you have to be at least 22 to get them (that may have changed, but it was true in the past); (ii) you will need to get a VERY occasional MRI to see whether the implant has ruptured (with saline it's obvious); (iii) the incision is always bigger with silicone, because with saline they fill the implant after it's inserted, so the rolled up saline implant only needs a tiny incision; and (iv) you are stuck with the size silicone implants that a manufacturer produces.

Saline does have some benefits. For one, saline implants are filled after insertion, so a Dr. can fill them, see what they look like, and then add more saline solution if they need to be a bit bigger, and a woman can also get a precise size (for example, silicone implants may only come in 350 and 450 cc's, but she may want 400). You can also fill the two implants with different amounts of saline solution, which helps if a chick has slightly lopsided tits. Also, for women who prefer a faker look (and some do), saline implants can be "overfilled" (above the manufacturers recommendation) giving a very firm and rounder look. Saline implants are certainly more versatile.

Size is pretty self explanatory. Understand though that because an implant is more spherical it will tend to look smaller for its cup size than a natural pair of tits.Natural Ds will ALWAYS look bigger than fake Ds. So chicks need to keep in mind that if they like the look of natural Ds, they should probably get fake DDs.

Also, and pretty obviously, a bigger chick will need to take a bigger implant. A girl who is 5'10 and 140 will swallow up a smaller set of implants, and imo one of the worst things a chick can do is go through the trouble of getting implants and then not have any effect. She should always err a tiny bit bigger.
