Are these alleged celebrity NDAs real and actually enforced?

Allegedly Lebron James and Leonardo DiCaprio and others have sex with women and make them sign NDAs.
Or Hilary Duff throws a cell phone at her assistants head and pays her off and makes her sign an NDA. Even reality TV shows have these huge penalties for millions if a participant reveals things.
Am I wrong, or are these totally unenforceable from a practical standpoint? Like if some chick says she slept with Lebron James, he's going to sue her (and basically admit it)? And then the court is probably going to just tell her to stop - or maybe he wins monetary damages and gets possession of like her Kia?

it's very common.
yesterday, Chris Hansen went to the A24 studios to watch an early cut of the new movie about To Catch a Predator. Chris had no say in any aspect of the movie, but they wanted him to sign an insane NDA before getting to see the movie.
