Basically, if there isn't some exigent circumstance like the PM dies in office or becomes a raging demented alcoholic, they are removed by order of the monarch. The monarch typically orders the PM to "meet with me" and they have a closed door meeting, during which the PM gracefully resigns, but there are no hard rules that require it.
